Information clause

In accordance with Art. ,13 of the General Data Protection Regulation of 27 April, 2016, (Journal of Laws UE L 199 of 04.05.2016), I hereby inform that:

1) providing personal data is  necessary for the conclusion and execution of the contract

2) Uzdrowisko Cieplice Sp. z o.o. – Grupa PGU is the administrator of your personal data z o.o.  – Grupa PGU

3) Contact with the Data Protection Officer is possible via e-mail at : iod@uzdrowisko-cieplice.pl

4)Your personal data will be processed in order to conclude and provide tourist, accommodation, therapeutic and health services   – pursuant to Art. 6, paragraph 1 letter b) of the General Data Protection Regulation of 2016 April, 2016,

5) the recipients of your personal data will include only entities authorized to obtain personal data on the basis of legal provisions, as well as entities participating in the provision of services

6) Your personal data will be stored for the period necessary to achieve this goal

7) You have the right to request the administrator to grant you access to your personal data, to modify them, delete them or limit the processing of them, as well as the right to transfer your data;

8) You have the right to lodge a complaint with the supervisory authority – the President of the Office for Personal Data Protection.
